The way it works is simple: complete an eligible activity* and get rewarded with a gift card.

Go to Meridian.Healthmine.com to get started. You will need your Member ID Card, date of birth, and email address to set up your account.

  2. Choose your preferences.
  3. Create a password.
  4. Ready to go!
